What the forms ask for

The enquiry form asks for your name, email address, role, support preference and consent to contact. A phone number and brief message are optional. The referral form also offers an optional participant first name and asks you to confirm permission to make the referral.

Please leave NDIS numbers, diagnoses, medical histories, plan documents and other sensitive records out of these forms. We can discuss a suitable way to share relevant information later, with the person’s consent.

How we use an enquiry

We use the details you provide to respond to the enquiry, clarify the requested service and discuss appropriate next steps. Sending an enquiry does not start a service agreement or determine NDIS funding.

Website forms are handled by Netlify, our website hosting and form service. Its processing is subject to the Netlify privacy policy. Access to enquiries should be limited to people handling the requested support. We do not use these forms to sign you up for marketing.

Website tools and technical information

The support finder keeps its choices in the current page’s memory. It does not save or submit your answers. The resource search filters guides in your browser.

This website does not add advertising trackers or analytics cookies. Fonts and photographic assets are hosted with the site. The hosting service may process technical request information, such as an IP address, to deliver and protect the website.

External links open a separate website whose own privacy practices apply. Email enquiries are also handled by the email services involved in sending and receiving the message.

Your choices and questions

You can choose to email us directly instead of using a form. To ask about the information held from your enquiry, request a correction or deletion, withdraw consent to further contact, or raise a privacy concern, email jennofrie@supportstreamsolutions.com.au. Relevant record-keeping obligations may affect whether particular information can be deleted.
